According to the Direction of the European Council No. 1346/2000 dated 29th of May 2000, as the bankruptcy trustee of the debtor, I am obliged to inform you that with the resolution of the District Court Nitra, Proc. No. 32OdK/278/2023, dated the 02rd of January  2024 bankruptcy procedure was declared on the Debtor, Marián Stehlík, born 16^th September 1971, 395 84 Horné Semerovce 216, Slovak republic, operating under the business name Marián Stehlík, ID No.: 44 793 693, with place of business 935 84 Horné Semerovce 216, with terminated business activity as of 20.01.2010.

V zmysle zákona č. 7/2005 Z. z. o konkurze a reštrukturalizácii v znení neskorších predpisov (ďalej len „ZKR“) veritelia dlžníka sú povinní v lehote 45 dní od vyhlásenia konkurzu prihlásiť svoje pohľadávky v jednom rovnopise správcovi na adresu:

LexCreditor k.s. so sídlom kancelárie: Laurinská 3/A, 811 01 Bratislava, Slovenská republika.

Pre každú pohľadávku musí byť predložená samostatná prihláška. V prihláške sa vždy uvedie meno, priezvisko a bydlisko alebo názov a sídlo veriteľa a dlžníka, právny dôvod vzniku pohľadávky, poradie

uspokojenia pohľadávky a suma pohľadávky s rozdelením na istinu a príslušenstvo, a príslušenstvo s rozdelením podľa právneho dôvodu vzniku; prihláška musí byť datovaná a podpísaná veriteľom.

Rovnakým spôsobom uplatňujú svoje pohľadávky aj veritelia pohľadávok, ktorých vznik je viazaný na

splnenie podmienky, pričom sa uvedie podmienka, od ktorej závisí vznik pohľadávky, a aj veritelia pohľadávok zabezpečených vecným právom, ktorí v prihláške uvedú druh, poradie a právny dôvod vzniku vecného práva spolu s určením majetku, ktorým je zabezpečená a sumu, do ktorej je zabezpečená. Pohľadávka sa uplatňuje v mene euro. K prihláške sa pripájajú listiny preukazujúce údaje uvedené v prihláške. Ak veriteľ nemá na území Slovenskej republiky bydlisko alebo sídlo alebo

organizačnú zložku podniku, v prihláške je povinný uviesť zástupcu na doručovanie písomností, ktorý

má bydlisko alebo sídlo na území Slovenskej republiky. Ak sa prihlasuje nepeňažná pohľadávka, k prihláške sa pripojí aj znalecký posudok určujúci hodnotu nepeňažnej pohľadávky.

According to the Act No. 7/2005 Coll. Bankruptcy and Restructuring Act as amended (hereinafter only

„the BRA”) the creditors of the debtor have to lodge their claims in a time period of 45 days beginning with the declaration of the bankruptcy procedure in one original to the bankruptcy trustee to the address:

Each claim must be lodged separately. The Iodgement of claim has to provide information about the name, surname and the address of the firm and the seat of the creditor and of the debtor, the legal cause of the claim, ranking of the satisfaction of the claim from the debtor’s estate and the amount of the principal and the interests, the legal cause of the interests; the lodgement of claim has to be dated and signed.

Creditors whose claims depend on the fulfilment of certain conditions lodge their claims in the same way. They also state in the registration form the condition on which the claim is dependent. Creditors with claims secured with securities also fill in the registration form which has to provide information about the object to which the security is tied. The claim has to be lodged in currency named as EUR. Documents proving the information provided in the lodgement of claim have to be enclosed to the lodgement of claim. In case the creditor does not have a seat or an address or a branch office in the Slovak Republic a representative with an address or a seat in the Slovak Republic has to be stated. In case a non-financial claim is lodged, an expert opinion stating the value of the claim has to be added.

Na prihlášky, ktoré nebudú spĺňať zákonom stanovené podmienky, nebudú podpísané a datované, alebo nebudú doručené v stanovenej lehote sa v konkurznom konaní neprihliada. Neprihlásené zabezpečovacie právo uplynutím lehoty na prihlasovanie pohľadávok zanikne. Správca ani súd nie sú povinní vyzvať veriteľa na doplnenie alebo opravu neúplnej alebo nesprávnej prihlášky.

Lodgement of claims that will not fulfil the requirements stated by the law or will be not signed or dated or an expert opinion (in case of non-financial claim) will not be attached to the lodgement of claim or lodgement of claim which will not be delivered in time will not be considered as claims in the bankruptcy procedure. Not lodged security rights become extinct after the lodgement period elapses. The trustee or the court do not have the duty to notify the creditor to amend or to correct the incorrect or the incomplete lodgement of claim.

Veriteľ zodpovedá za správnosť údajov uvedených v prihláške podľa § 30 ZKR.

The creditor is responsible for the legitimacy of his registration according to the article 30 of the BRA.
